I would first like to congratulate you on the next step of your life. This can be a challenging time trying to apply to schools and needing letters of recommendations. I appreciate you asking me. I understand the process can be different for every institution so please follow the directions below to make the process go as smooth and quick as possible.
PLEASE NOTE: I need two weeks for all recommendation requests.
Step 1: Fill out the Google Form: Request for Letter of Recommendation next to this sentence. After completion please see Mr. Davis before step 2.

Step 2: Fill out the Google Form: Student Questionnaire Form. Please submit the form and it will notify me you have completed it.
Please note that there is a lot of time that goes into writing your college recommendation. I would like to do my very best when I am writing your recommendations and this requires to know more information about you. Please give these questions some thought. You want me to understand all aspects of your school life so that I can highlight the person you are. This questionnaire is intended only for my use. If at any time you are not sure what to do, please email me or see me in person.

Step 3: Please notify me in the Google Form if you need paper copies of your letter of recommendation or if you need me to use the Common app.

As always, please know that open communication is important through this process. Please feel free to contact me if you have any questions.
Any student who request a copy of the letter of recommendation will receive the letter in a sealed envelope.
